Prevenirea si managementul infectiilor postsplenectomie in trauma lienala

REZUMAT

La ora actuală, splina este considerată principalul organ imun periferic. Infecţiile sunt cele mai importante consecinţe ale absenţei splinei, acestea putând avea o evoluţie severă către sepsis (OPSI=Overwhelming Post Splenectomy Infections/sepsisul fulminant postsplenectomie) şi deces. De aceea, în contextul traumatismelor şi nu numai, conservarea splinei ori efectuarea transplantului autolog lienal se impun pentru prevenirea infecţiilor postsplenectomie. În plus, pacienţii necesită imunoprofilaxie (vaccinare pneumococică, vaccinare Haemophilus influenzae tip b şi vaccinarea meningococică) şi chemoprofilaxie (penicilina V/amoxicilină).
